"How do you know for sure?" -- good question.
I did read once that nowadays you need more than one virus engine. On that article, there were a few charts that showed how quick numerous AV companies were to learn new virii and deploy the definitions, and throughout the charts, all the companies had different positions (which corresponded to how quick they were to respond to new virii) on the charts.
I guess you can't really know if an AV really works, but if you play safe on the Internet, then you should be pretty confident that the AV did do its work.

Good self control on the internet and email is surprisingly effective at keeping this stuff off your system. Not 100% but if you watch where links are taking you and stay away from anything you feel is dodgy. Closing a popup without interacting with it to do so. A bit anal but my system boots and shuts down as quick as it did when I first built it several years ago now.
