Hi all, this is a wild shot in the dark and could have been mentioned in the above links I didn't read cause I'm just killing time update my linux box, but
the comment about "hitting F5 and the icon appearing" is the exact same symptom
I've had to deal with in our environment. Anyways -->
Just refresh the icon cache by deleting the IconCache.db file from your profile directory (usually /Documents and Settings/Username/Local Settings/Application Data). It will be automatically recreated.
if this works you can edit the registry IconCache size here --> HKEY_LOCAL_MACHINE/Software/Microsoft/Windows/CurrentVersion/Explorer. In the right pane, look for the value named Max Cached Icons, change this to 2048 or higher if you wish but within a sensible range please : )
